2008 COMMUNITY INVESTMENTS APPLICATION
Speaking of Women’s Health (SWH)
with the support of its Corporate Sponsors annually awards
a COMMUNITY
INVESTMENT GRANT to a non-profit 501(c) (3) organization.
WPBT Channel 2, on behalf of the Steering Committee, is accepting
applications from non-profit organizations for the 2008
Speaking of Women’s Health Community Investments Grant .
The funds are available to support non-profit organizations,
which educate women on their health, well-being and personal
safety.
The Speaking of Women’s Health
Conference will be held November 22, 2008
at
the Signature Grand in Davie
, Florida .
The Steering Committee of Speaking of Women’s
Health Conference will consider applications that
are educational and pertain to the health and well-being
of women and/or enhance the quality of women’s lives.
Funds for start-up organizations will not be granted. Organizations
need to have a board and budget before being considered
for Community Investment Awards.
To apply for the Community Investment Grant, please
complete and return the following application and 7 copies
of the materials, no later than August 29, 2008.
(All requested information must be turned in as a packet
to be considered for funding. Please send 7 copies of all
materials.)

3. A copy of the annual operating budget
4. A list of board
members
5. A copy of the organization’s Annual Report,
and/or Executive Summary or Bio
6. List other grants & awards
you have received
7. Summary of the following (should not
exceed 2 pages):
• 3 reasons why it is important to support your
program
• 3 ways in which your program enhances
the health and well being and lives of women
• A
brief explanation of how the Speaking of
Women’s Health Community Investment Grant would
be used
